超声二维应变成像评价2型糖尿病患者左右心室局部收缩功能的研究

Evaluation of Left and Right Ventricular Regional Systolic Function in Patients with Type 2 Diabetes Mellitus by Two-Dimensional Strain Imaging

摘要 目的应用超声二维应变成像评价2型糖尿病患者左、右心室局部收缩功能。方法选取2型糖尿病患者30例(T2DM组)和健康者25例(正常对照组)。先对两组受检者进行常规二维超声心动图检查,然后在心尖四腔切面测量两组受检者左心室侧壁及右心室游离壁基底段、中间段、心尖段心内膜下心肌纵向收缩期二维应变峰值。结果 T2DM组和正常对照组二维超声心动图参数比较,差异无统计学意义(P>0.05)。两组受检者左心室心肌长轴收缩期峰值应变从基底段至心尖段呈逐渐增大的趋势,而右心室心肌长轴收缩期峰值应变从基底段至心尖段呈逐渐减低的趋势;T2DM患者左、右心室心内膜下纵向应变收缩期峰值较正常对照组显著降低,各个节段间差异均有统计学意义(P<0.05)。结论超声二维应变成像可以发现2型糖尿病患者在心脏整体收缩功能出现降低前左右心室局部心肌收缩功能已降低。 Objective Using two- dimensional strain imaging to assess the left and right ventricular regional systolic function in patients with type 2 diabetes mellitus. Methods 30 type 2 diabetes mellitus (T2DM group) and 25 healthy subjects (control group) were chosen. First, the normal two -dimensional echocardiography were performed on the two groups. And then, two dimensional strain were performed in measuring the peak systolic longitudinal strain of the subendocardial myocardium in the apical 4 - chamber views by the left ventricular lateral wall and right ventricular free wall in mitral, basal, mid, apical seg- ments. Results There was no statistical difference between the two groups in using the normal two - dimensional echocardiography (P 〉0. 05) . In the two groups, the peak systolic longitudinal strain had the tendency of increasing from the base to apical seg- ments, wheras the tendency of decreasing from the apical to base segments; The peak systolic longitudinal strain of the subendo- cardial myocardium of different segments in patients with type 2 diabetes mellitus was significantly reduced when compared with the control group (P 〈 0. 05 ) . Conclusion The systolic function of subendocardial myocardium evaluated by two - dimensional strain has been reduced before the left ventricular global systolic function reduces in patients with type 2 diabetes mellitus.
