乙型肝炎病毒感染者HLADRB基因分型及其相关性研究

HLA-DRB genotyping and its relation in patients with HBV infection

目的探索乙肝病毒(HBV)感染者HLA DRB基因频率及其与疾病的相关性。方法用PCR SSP对重庆地区汉族乙肝患者40例和正常人120名进行HLA DRB基因分型 ,结合临床病型与HBV复制状态作相关性分析。结果重庆地区汉族正常人以DR2(15)、DR5(12)、DR9 位点常见 ,乙肝患者与之比较DR3 基因频率明显增高(P<0.05) ,DR5(12)、DR7 基因频率也有增高 ,均见于慢性乙肝和肝硬化 ,相对危险度DR3 为2.4,DR7 为2.0。DR7基因多见于HBV高复制状态 ,相对危险度为2.4。结论HLA DR3 基因与重庆地区汉族慢性乙肝和肝硬化相关 ,DR7 与HBV高复制状态有关 ;免疫遗传因素参与慢性乙肝的发病机制。

Objective To study the frequency of different genotypes of HLA-DRB gene and their relations to HBV infection. Methods HLA-DRB genotyping was conducted in 40 cases of HBV infection and 120 healthy subjects with polymerase chian reaction-sequence specific primer (PCR-SSP). All the patients and healthy subjects were residents of chongqing district and belonged to Han Natiionality. The relationship of different genotypes of HLA-DRB with the clinical types of HBV infection was analyzed. Results HLA-DR2(15), DR5(12) and DR9 were the frequent genotypes in the healthy residents of Chongqing with the frequency of 18.3%, 16.7% and 14.2% respectively. The frequency of HLA-DR3 was increased from 3.3% in healthy subjects to 12.5% in patients with chronic hepatitis B and hepatitis B-associated liver cirrhosis (P<0.05). The frequency of HLA-DR5(12) and HLA-DR7 was also increased in those patients. The relative risk coefficient was 2.4 for HLA-DR3 and 2.0 for HLA-DR7. HLA-DR7 was usually found in high replication condition of HBV with a relative risk coefficient of 2.4. Conclusion HLA-DR3 is related to hepatitis B and liver cirrhosis of the Han Nationality in Chongqing district. HLA-DR7 is usually found in high replication condition of HBV. And immune and genetic factors participate in the pathogenesis of hepatitis B.
