摘要
目的探讨骨肉瘤MDM2和P53蛋白定位 ,表达水平 ,二者的相互关系及其对骨肉瘤转移及预后的影响。方法应用免疫组织化学链菌素亲生物素蛋白过氧化酶连接法(S P法) ,检测80例有随访资料的骨肉瘤MDM2和P53蛋白的表达 ,比较其与各病理指标之间的关系。结果①骨肉瘤组织MDM2蛋白总体阳性率为50 %(40/80) ,P53蛋白总体阳性率为40%(32/80) ;MDM2蛋白和P53蛋白的异常表达呈显著负相关(P<0.01)。②骨肉瘤MDM2和P53蛋白表达与年龄、性别、肿瘤大小、Price分级及Dahlin分型均无关 ,而与骨肉瘤WHO新分型有关(P<0.05) ,MDM2蛋白表达与肿瘤的复发或转移密切相关(P<0.05)。③生存资料分析显示 :MDM2或P53蛋白阳性表达组生存率明显低于阴性组(P<0.01) ;MDM2和P53蛋白同时阳性表达者生存率最低 ,而二者共同阴性者生存率最高 ,两组相差极为显著(P<0.005)。结论①MDM2蛋白检测可能应用于预测骨肉瘤有无复发或转移 ;②用免疫组化方法联合检测MDM2和P53蛋白表达可能成为评价骨肉瘤预后的一项有价值指标。
Objective To investigate the expression of MDM2 and P53 protein in osteosarcoma and its relation to the clinical-pathological parameters and prognosis of the tumor.Method The expression of MDM2 and P53 protein was detected immunohistochemically in specimens from 80 cases of osteosarcoma with follow-up data. Results ①The positive rates P53 and MDM2 were 40% and 50% respectively. There was inverse relationship between the expression of P53 protein and that of MDM2 (P< 0.01). ②P53 protein and MDM2 were not significantly correlated to the sex, age, tumor size, grading or Dahlin's classification of the tumor. The expression of MDM2 was significantly correlated with tumor's recurrence and metastasis. ③Analysis of survival data showed that patients with positive reaction for both antibodies had the lowest survival rate and the poorest prognosis.Conclusion The detection of MDM2 can be used to predict the tumor's recurrence and metastasis and combined detection of MDM2 and P53 is valuable in evaluating the prognosis of osteosarcoma.
