累及胃肠道的间变性大细胞淋巴瘤的临床病理学特点

Clinicopathologic Features of Gastrointestinal Tract Involvement of Anaplastic Large Cell Lymphoma

目的总结累及胃肠道的间变性大细胞淋巴瘤(ALCL)的临床病理学特点。方法收集整理4例累及胃肠道的ALCL病例标本,对其进行临床病理分析、免疫组织化学检测、EB病毒编码的RNA(EBER)原位杂交检测及T细胞受体(TCR)基因重排检测。结果 4例病例中,肿瘤细胞均以多形性大细胞为主,归为普通型ALCL;均表现为弥漫、较强地表达CD30,且至少表达1种细胞毒性抗原;2例为间变性淋巴瘤激酶(ALK)阳性ALCL;4例EBER原位杂交检测均为阴性且均存在TCR基因克隆性重排。结论累及胃肠道的ALCL具有独特的临床病理特点。

Objective To investigate the clinicopathological characteristics of gastrointestinal tract in- volvement of anaplastie large cell lymphoma （ALCL）. Methods The clinicopathological features of four pa- tients with ALCL that involved gastrointestinal tract were retrospectively analyzed using immunohistochemical study, T-cell receptor gene rearrangement analysis, and evaluation for Epstein Barr virus infection status. Re- sults Most tumor cells in all these four cases are large and highly pleomorphic, and all four cases were classi- fied as the ＂common pattern＂ ALCL. Tumor cells in all four tumors expressed CD30, and expressed at least one cytotoxic maker. Two patients were confirmed to be with anaplastic lymphoma kinase （ALK） -positive ALCL, and four patients were negative during in situ hybridization for Epstein-Barr virus-encoded RNA but showed clonal T-cell receptor gene rearrangement. Conclusion Gastrointestinal tract involvement of ALCL has the unique clinicopathological features.
