结节性胶样变性

Nodular colloid degeneration

报告1例结节性胶样变性。患者女,53岁。因面部起半透明的丘疹、结节和斑块伴轻微瘙痒8年就诊。组织病理检查显示真皮全层可见团块状淡红色无定形的物质,内有许多裂隙和散在分布的成纤维细胞。PAS染色阳性,刚果红染色阴性。弹性纤维染色示胶样物质团块内和周围有团块状变性的弹性纤维。符合结节性胶样变性的诊断。组织病理上该病需与成人胶样粟丘疹、结节性皮肤淀粉样变性及进行性获得性淋巴管瘤等鉴别。

A ease of nodular colloid degeneration is reported. A 53-year-old woman presented with a 8-year history of slightly pruritic and translucent papules, nodules and plaques on her face. Histological examination showed large, amorphous masses of slightly eosinophilic material in the whole dermis. Fissures and clefts divided the material into smaller islands. Fibroblasts were observed within these amorphous deposits. PAS stain was positive. Congo red stain was negative. Elastic fibers stain demonstrated coarse elastie fiber within the amorphous mass and in the surrounding dermis. These findings were consistent with nodular colloid degeneration. Differential diagnosis must be made with adult colloid milium, nodular cutaneous amyloidosis, and acquired progressive lymphoangioma in histopathology.
