多层螺旋CT扫描与低场磁共振MRCP技术在诊断胆道梗阻性疾病中的应用价值

摘要 目的:探讨多层螺旋CT三维重建技术与低场磁共振MRCP技术在诊断胆道梗阻性疾病中的应用价值对照。方法:应用多层螺旋CT与低场磁共振MRCP技术对46例超声诊断为胆道梗阻患者进行上腹部薄层平扫及多期薄层增强扫描,并用薄层增强的扫描原始数据行三维胆系重建,对胆道梗阻行定位及定性诊断与低场磁共振MRCP检查结果比对,均经术后病理证实,回顾性分析行多层螺旋CT与低场磁共振MRCP检查的46例梗阻性黄疸病患者的临床资料。结果:在定位诊断方面,多层螺旋CT薄层平扫+多期薄层增强扫描结合三维胆系重建,与低场磁共振MRCP技术在胆道梗阻性疾病中定位诊断准确率均100%;在定性诊断方面,对良性病变尤其是对结石的诊断,前者高于后者。结论:多层螺旋CT对胆道梗阻的定位、定性诊断率高,是诊断胆道梗阻的重要方法之一;而MRI对胆道梗阻性疾病定位诊断准确,定性诊断也更具有较高的特异性,是一种安全快捷、非创伤性的检查方法,能为胆道梗阻的诊治及术后评估提供可靠的依据。 Objective :To investigate the multi - slice spiral CT three - dimensional reconstruction technique with low field magnetic resonance imaging of MRCP in the diagnosis of biliary obstructive disease application value control. Methods:muhi - slice spiral CT with low field magnetic resonance MRCP technique46 cases ultrasound diagnosis of biliary obstruction in patients with abdominal thin layer scan and multi phase enhanced scanning and thin, with thin layer scanning the original data enhanced three - dimensional biliary reconstruction of biliary obstruction, localization and qualitative diagnosis of low field magnetic resonance MRCP examination results,were confirmed by postoperative pathology confirmed, retrospective analysis of multilayer spiral CT with low field magnetic resonance MRCP examination of 46 cases of obstructive jaundice patient clinical data. Results:in the localization diagnosis of multislice spiral CT scan, thin layer + multi period of thin - slice scanning with 3 -D biliary reconstruction, and low field magnetic resonance MRCP technique in biliary obstructive disease diagnosis rate was 100%;in the qualitative diagnosis of benign lesions, particularly for the diagnosis of gallstone, the former is better than the latter. Conclusion: multi - slice spiral CT in the localization of biliary obstruction, qualitative diagnosis rate is high, is one of the important methods in diagnosis of biliary obstruction;and MRI in the diagnosis of biliary obstructive disease, qualitative diagnosis also has a high specificity, is a safe, fast, non -invasive examination method, for biliary obstruction and postoperative assessment provides the reliable basis.
