不同宫颈锥切方法治疗宫颈上皮内瘤变Ⅲ级的临床疗效评价

目的评价不同的宫颈锥切方法治疗宫颈上皮内瘤变Ⅲ级的临床疗效。方法对确诊为宫颈上皮内瘤变Ⅲ级的患者临床资料进行回顾性分析,其中100例施行宫颈冷刀锥切术(CKC),100例施行宫颈环形电切术(LEEP),比较不同宫颈锥切方法的临床疗效。结果两组手术方法的临床疗效基本相同,但是LEEP组的手术时间和术中出血量明显低于CKC组([6±3)min比(30±6)min,(8±4)ml比(50±14)ml,均P<0.05];CKC组的病情复发率和术后高危HPV持续感染率明显低于LEEP组,两组比较差异有统计学意义(P<0.05)。结论在治疗宫颈上皮内瘤变Ⅲ级过程中,CKC治疗可以有效减少病情复发及预防高危HPV的持续感染;而LEEP治疗疾病时,患者的创伤小、痛苦少,且具有手术时间短和术中出血少的优点,是治疗宫颈上皮内瘤变Ⅲ级安全、有效的方法。

Objective To evaluate clinical efficacy of different cervical conization in treatment of cervical intraepithelial neoplasia grade Ⅲ Methods The clinical data of cervical intraepithelial neoplasia grade III were retrospectively analyzed, 100 patients underwent cervical cold knife conization （CKC）, 100 patients underwent cervical loop electrosurgical excision procedure （LEEP）, the clinical efficacy of different cervical conization method were compared. Results The clinical efficacy of the two groups of surgical methods are basically the same, but the surgery of the cervical loop electrosurgical excision procedure time and the book amount of bleeding was significantly lower than the cervical cold knife conization, cervical cold knife conization surgery relapse rate and postoperative high risk HPV continuing infection rate was significantly lower than the cervical loop electrosurgical excision procedure. The difference was statistically significant （P〈0.05）. Conclusion In the treatment of cervical intraepithelial neoplasia grade Ⅲ, cervical cold knife conization treatment is effective in reducing relapse and prevention of persistent infection with high risk HPV; the treatment of diseases of the cervical loop electrosurgical excision procedure for patients with trauma and less pain, and has the advantages of shorter operative time and less blood loss, the treatment of cervical intraepithelial neoplasia grade Ⅲ is safe and effective method.
