摘要
目的 探讨幽门螺杆菌 (Hp)感染对胃上皮细胞凋亡及调节蛋白表达的影响。方法 应用原位末端标记法及免疫组织化学技术对 37例Hp阳性慢性胃炎和 31例Hp阴性慢性胃炎胃黏膜上皮细胞凋亡指数及p5 3、bcl 2、Bax、Fas蛋白表达进行观察和比较。结果 Hp阳性慢性胃炎胃上皮细胞凋亡指数显著高于Hp阴性慢性胃炎 (P <0 0 0 1) ,Hp阳性慢性胃炎上皮p5 3表达率显著高于Hp阴性者 (P <0 0 0 5 ) ,而bcl 2表达率则显著低于Hp阴性慢性胃炎者 (P <0 0 5 ) ,但Bax、Fas蛋白表达率之间 2组差异无显著性。结论 Hp感染可诱导胃上皮细胞凋亡并导致p5 3蛋白积累 ,增加了基因不稳定性 ,尚可抑制bcl 2表达 ,促进凋亡发生 ,Hp感染诱导的凋亡不依赖Bax、Fas途径。
Objective To study the effect of Helicobacter pylori (Hp)infection on gastric epithelial cell apoptosis and oncogen expression Methods Gastric mucosal biopsy specimens were taken from 37 patients with Hp positive chronic gastritis and 31 with Hp negative chronic gastritis Gastric epithelial cell apoptosis and p53, bcl 2,Bax,Fas proteins expression were detected using immunohistochemical staining and terminal uridine deoxynucleotide nick end labeling (TUNEL) technique Results The apoptotic index(AI)and the positive rate of p53 protein in patients with Hp positive was significantly higher than in patients with Hp negative( P <0 001 , P <0 005, respectively) But the positive rate of bcl 2 protein was opposite ( P <0 05) There were no difference in the expression rates of Bax and Fas protein between Hp positive and Hp negative Conclusions Hp infection could induce gastric epithelial apoptosis and it occurs through a mechanism that is independent of Bax, Fas gene pathway Hp infection results in the accumulation of p53 protein enhancing the genomic instability and inhibits the expression of bcl 2 protein
