163例柯萨奇病毒B组肺炎临床分析

Clinical analysis of 163 children with coxsackie virus B pneumonia

为探讨柯萨奇病毒(B组)所致肺炎的患病率、临床特点及干扰素治疗疗效,于1995年1月至1996年6月在呼吸科住院的1211例肺炎患儿中,随机检测CVB抗体843例,其中163例阳性,阳性率19.34％。与AdV、RSV肺炎比较,CVB肺炎的特点为住院前病程长,中毒症状轻,多干咳,少痰,呼吸急促少见,住院时间较长,伴疱疹性咽峡炎和肌痛,偶有腹泻和皮疹。实验室检查以乳酸脱氢酶及其同功酶异常最显著。随访2个月X线仍异常,异常率为35.29％,提示病程迁延。干扰素治疗组在入院后热程、咳嗽消失时间、住院天数、肺部罗音吸收和胸部X线炎症吸收时间等方面比对照组有明显缩短,前3项有显著差异(P<0.05),后2项其差异非常显著(P<0.01)。

In order to investigate the incidence and clinical features of coxsackie virus B pneumonia (CVB), and therapeutic effects of interferon for CVB, CVB antibody was randomly detected in 843 cases from 1211 hospitalized patients during Jan. , 1995 to Jun. , 1996. The results showed that the positive rate of CVB antibody was 19. 34% (163/843). Comparing with respiratory syncytial virus (RSV) and adenovirus (AdV) pneumonias, the disease course of CVB patients were longer before admission and toxemic symptoms were not serious. It usually menifestrated herpangina, myodynia, and dry cough without sputum and tachypnea, rarely diarrhea and rash. Abnormal LDH isozyme could be found in the laboratory examination. There was a protracted course of disease, longer hospitalization, and with 35. 29% of abnormal X-ray finding within two months. The fever and cough course, hospitalization days, the time of rales absorption in the lungs and inflammation absorption in chest X-ray shortened significantly in the patients who received interferon treatment(P<0. 05 or 0. 01).
