回声强度定量分析在腮腺腺淋巴瘤与混合瘤鉴别诊断中的价值

The value of quantitative analysis of sonographic echo intensity in differentiating pariotid adenolymphoma from pleomorphic adenoma

目的探讨回声强度定量分析在腮腺腺淋巴瘤与混合瘤鉴别诊断中的价值。方法收集病理证实的15例腺淋巴瘤和12例混合瘤的超声资料,经过影像归档和通信系统(PACS)在医学平板显示器上观察,利用自由兴趣区绘画工具测量并计算病变相关指标。结果病变内部及其后方回声强度值与病变最大径线无相关性(P>0.05)。腮腺腺淋巴瘤的内部回声强度值低于混合瘤(P<0.05),腺淋巴瘤内部回声强度值与正常腮腺回声强度值的比值也低于混合瘤(P<0.05)。ROC曲线显示病变内部回声强度、病变内部回声强度与正常腮腺回声强度的比值在诊断混合瘤的曲线下面积分别为0.772(P<0.05)和0.783(P<0.05)。结论超声回声强度定量分析有助于腮腺腺淋巴瘤和混合瘤的鉴别诊断。

Objective To investigate the value of quantitative analysis of sonographic echo intensity（SEI） in differentiating pariotid adenolymphoma from pleomorphic adenoma. Methods The SEI was quantitatively analyzed with picture archiving and communication systems in 15 pariotid adenolymphomas（group A） and 12 pleomorphic adenomas （group B）, which were confirmed by pathology. Results Both the tumor signal value and the ratio of the signal value of pariotid adenolymphoma to normal parotid were significantly lower in group A than those in group B （P〈0. 05）. The area under the ROC curve of tumor signal value and the ratio of the tumor signal value to normal parotid in differentiating ple0morphic adenoma from pariotid adenolymphoma were O. 772 and 0. 783 （P〈0. 05）, respectively. Conclusion The quantitative analysis of SEI is useful in differentiating the pariotid adenolymphoma from pleomorphic adenoma.
