Herbert镙钉治疗舟状骨骨折的临床效果分析

Efficacy analysis on Herbert screw internal fixation for carpal scaphoid fracture

目的探讨Herbert镙钉在治疗腕舟状骨骨折中的应用效果。方法选取2008年10月～2010年10月在本科采用Herbert螺钉内固定手术治疗的腕舟骨骨折患者16例,术后第12个月按照Cooney评分法对腕关节功能进行评价。结果随访6～16个月之后,所有患者骨折均达到骨性愈合标准。按照Cooney腕关节功能评分,术前:优0例,良2例,可3例,差11例,优良率为12.5%。术后第12个月:优9例,良5例,可2例,差0例,优良率为87.5%,治疗前后优良率比较差异有高度统计学意义(P<0.01)。结论 Herbert螺钉用于腕舟骨骨折,固定牢固,有效地缩短治疗周期,骨不愈合发生率低,更好地恢复了腕关节功能,临床效果满意。

Objective To study the clinical effect of carpal scaphoid fracture treated with Herbert screw fixation. Methods Sixteen carpal scaphoid fracture patients who were treated with Herbert screw internal fixation in our department from Oc- tober 2008 to October 2010 were retrospectively analyzed. The function of wrist was evaluated according to Cooney func- tional grading system at the 12th month after surgery. Results All patients achieved bone healing standard during a follow- up apriod of 6-16 months. According to Cooney functional grading system, no case was excellent, 2 cases were good, 3 cases were fair, 11 cases were p^Sgr before the operation, the excellent and good rate was 12.5%. Twelve months after the operation, 9 cases was excellent, 5 cases were good, 2 cases were fair, no case was poor, the excellent and good rate was 87.5%, the excellent and good rates before and after the operation had significant difference （P 〈 0.01）. Conclusion Treat- ment of carpal scaphoid fracture by Herbert screw internal fixation can provide reliable fixation, effectively shorten the treatment time, reduce the incidence of nonunion and improve joint function, the clinical effect is satisfactory.
