经皮空心钉内固定配合桂枝汤加减治疗肱骨大结节骨折的疗效观察

Efficacy observation of the percutaneous cannulated screw internal fixation with Guizhi decoction minus on treatment of the greater tuberosity humerus fractures

目的观察闭合复位经皮空心钉内固定配合中药桂枝汤加减内服治疗肱骨大结节骨折的疗效。方法 2009年1月～2010年12月收治的肱骨大结节骨折患者(Neer分型中的二部分大结节骨折包括伴肩关节脱位的肱骨大结节骨折)均试行闭合复位经皮空心钉内固定治疗,术后均以中药桂枝汤加减内服。在手术后第3个月及6个月时进行Neer肩关节评分。结果 23例患者中所有患者骨折均临床愈合,平均时间6～8周,所有病例未发生骨折再移位、内固定松动、断裂、骨折不愈合、神经血管损伤等并发症。结论此种方法固定可靠,有利于患者早期功能恢复。

Objective To observe the efficacy of the percutaneous cannulated screw internal fixation with Guizhi decoction minus on treatment of the greater tuberosity humerus fractures. Methods Patients who were accepted from January 2009 to December 2010 （Neer classification-type two parts greater tuberosity fracture of greater tuberosity fracture of the humerus with shoulder dislocation） were closed reset by the skin cannulated screw fixation for the treatment. After the surgery, all of the patients were given Guizhi decoction for internal medicine. Neer shoulder score of 3 months and 6 months after surgery were recorded. Results All of 23 patients were clinically cured, with an average time of 6-8 weeks, without fracture dis- placement, loosening, fracture, fracture healing, complications such as neurovascular injury. Conclusion This method is fixed, reliable, and beneficial to patients with early functional recovery.
