体外冲击波治疗腰椎间盘突出症的临床观察

Clinical Observation on Extracorporeal Shock Wave in the Treatment of Lumbar Disc Herniation

目的观察体外冲击波疗法(extraeorporeal shock wave therapy,ESWT)对腰椎间盘突出症的疗效。方法应用国产体外冲击波骨科治疗机治疗腰椎间盘突出症83例,随机分为治疗组43例和对照组40例。治疗组以痛点为中心,行体外冲击波疗法;对照组行中频电疗、牵引、推拿等对症治疗。结果治疗后第4周,视觉模拟评分(VAS)评分,两组间比较差异无显著性意义。治疗后第8周和第12周治疗组有效率85%,对照组有效率73%,治疗组VAS评分明显低于对照组(P<0.05)。结论体外冲击波疗法可改善腰椎间盘突出症患者疼痛症状,是腰椎间盘突出症保守治疗的一种好方法。

Objective To evaluate the effect of extraeorporeal shock wave therapy （ESWT） on lumbar disc herniation. Methods 83 cases of lumbar disc herniation treated with ESWT were randomly divided into treatment group （43 cases） and control group （40 cases）. The treatment group received extraeorporeal shock wave therapy around the pain point and the control group received massage, drug and functional exercise. Results At the fourth week after treatment, VAS showed no significant difference between the two groups. However, VAS in the treatment group improved and ESWT was effective in 85% of the patients compared to 73% in the control group at the eighth and twelfth week （P 0.05）. Conclusions Extraeorporeal shock wave therapy may improve pain in patients with lumbar disc herniation, which is a valuble method for conservative treatment of lumbar disc herniation.
