高尿酸血症病人雌激素对糖代谢的影响 被引量:5

INFLUENCE OF ESTROGEN ON GLYCOMETABOLISM IN PATIENTS WITH HYPERURICEMIA

摘要 目的分析绝经前后高尿酸血症病人雌激素对糖代谢异常发生的影响。方法选取高尿酸血症病人(高尿酸血症组)315例,尿酸正常者(尿酸正常组)308例,每组又分为男性、绝经前女性和绝经后女性组,测定各组血尿酸、血糖、血脂、肝功能、肾功能、雌激素及胰岛素水平。结果高尿酸血症组糖代谢紊乱的发生率明显高于尿酸正常组(χ2=33.217~38.175,P<0.05);无论是高尿酸血症组还是尿酸正常组,绝经后女性糖代谢紊乱的发生率均明显高于绝经前女性(χ2=20.809~24.435,P<0.05)。高尿酸血症组绝经后女性糖代谢紊乱的发生率明显高于男性(χ2=4.432、4.459,P<0.05),尿酸正常组绝经后女性糖代谢紊乱的发生率与男性无明显差异。结论绝经后和绝经前女性雌激素水平下降可诱发或加重高尿酸血症引起的糖代谢紊乱。 Objective To analyze the effect of estrogen on abnormal glycometabolism (AGM) in patients before and after menostasis. Methods This study included 315 patients with hyperuricemia (HU group) and 308 with normal uric acid (NUA group). The patients in each group were then divided again into male group, pre-menopausal and post-menopausal female group. The blood uric acid, blood sugar, blood lipid, liver function test, estrogen and insulin in each group were measured. Results The incidence of AGM in HU group was obviously higher than that in NUA group (χ2= 33. 217-38. 175, P〈0.05) ; however, the incidence of AGM in postmenopausal women was dramatically higher than that in pre-menopausal women, regardless of whether they were in HU group or NUAgroup (χ2=20.809-24.435,P〈0.05). In HU group, the incidence of AGMinpostmenopausalwomen was much higher than that in men (χ2=4.432-4.459;P〈0.05). In NUA group, the difference of the incidence of AGM between postmenopausal women and men was not significant. Conclusion The decline of estrogen levels in both pre and post-menopausal women may induce or aggravate hyperuricemia, leading to glycometabolic disorder.
