摘要
目的 研究 2 0例正常人99m锝—二乙基乙酰基替苯胺亚氨二醋酸 (99mTc -EHIDA)肝胆动态功能显像定量分析 ,以便建立本实验室的正常值 ,为胆道功能性疾病的诊断提供帮助。方法 静脉注射99mTc -EHIDA后 ,用Genesys单光子发射计算机断层扫描仪 (SPECT)进行肝胆动态显像。结果 肝脏泌胆分数 (HEF)为 1 0 0 % ,半排时间 (Tex)为 (2 0 .8±6 .3)min ,高峰时间 (Tup)为 (4.6± 2 .7)min。胆囊排胆分数 (GBEF)为 (56.9± 1 8.5) % ,排胆期 (GBEP)为 (1 5± 7.4)min,排胆率 (GBER) 4 .2 %。正常胆系泌胆时相分析 :肝、胆囊、胆总管、胆管口括约肌及十二指肠胆汁出现时间分别为 (2 .3± 0 .7)min、(1 1 .7± 3.4)min、(1 3.5±3 .3)min、(2 2 .7±6 .3)min和 (2 3.5±8.2 )min;高峰时间分别为 (1 0 .5±2 .5)min、(40 .1±7.8)min、(2 2 .7±8.6)min、(45 .3± 8.8)min和(46.2±7.4)min ,5名受检者重复检查结果 ,相关性好 (r=0 .96) ,两均数差异无显著性意义(P >0 .0 5)。结论 99mTc -EHIDA肝胆动态显像定量分析 ,方法可靠 ,重复性好 ,不但能对胆汁排泄动力学研究提供胆汁排泄路径中各部位的功能特点 ,而且可以动态显示整个路径的形态 。
Objective The quantitative analysis of 99m Tc-EHIDA kinetic function imaging in 20 normal volunteers were done in order to set up normal values in the laboratory so that to provide aid to the diagnosis of functional diseases in the biliarytract.Method Liver biliary function imaging were done after injecting intravenously 99m Tc-EHIDA with Genesys SPECT.Results Function of liver-biliary secretion:HEF 100%, Tex (20.8±6.3)?min,Tup (4.6±2.7)?min,GBEF (56.9±18.5)%,GBEP (15±7.4)?min,GBER 4.2 %.Phase analysis of biliary excretion: radioactivity appearance time of liver, gallbladder, bile duct, Oddis sphincter and duodenum was (2.3±0.7)?min,(11.7±3.4)?min,(13.5±3.3) min,(22.7±6.3) min and (23.5±8.2) min respectively,peak time was ( 10.5 ±2.5) min,(40.1±7.8) min,(22.7±8.6) min,(45.3±8.8) min and (46.2±7.4) min respectively. The results of repeated test in 5 volunteers showed that there were good relationship(r=0.96)and no significant deference between two tests (P>0.05). Conclusions The method of quantitative analysis of 99m Tc-EHIDA hepato-biliary function imaging is reliable and of good reproducibility. It provides not only morphology but also function characteristics of liver biliary system and is of great significance in diagnosing the biliary tract disease.
