阿托伐他汀对慢性心力衰竭患者血浆脑钠肽及心功能的影响 被引量:12

Influence of atorvastatin on plasma brain natriuretic peptide and cardiac function in patient with chronic heart failure

摘要 目的观察阿托伐他汀治疗慢性心力衰竭后,血浆脑钠肽(BNP)水平的改变及心功能的改善情况。方法将心功能Ⅱ~Ⅳ级心力衰竭患者83例随机分为治疗组41例和对照组42例。对照组予常规抗心力衰竭(扩血管药物、利尿剂及洋地黄)治疗,治疗组在对照组治疗基础上加用阿托伐他汀。观察2组临床疗效和治疗前后BNP水平的变化及心功能的改变。结果治疗组总有效率为65.9%高于对照组的42.9%,差异有统计学意义(P<0.05)。治疗后,2组左室射血分数(LVEF)均有明显提高,BNP均有明显下降,且治疗组LVEF高于对照组、BNP低于对照组,差异均有统计学意义(P<0.05)。血浆BNP浓度与LVEF存在负相关(r=-0.167,P<0.01)。结论阿托伐他汀能改善慢性心力衰竭患者的心功能,增加LVEF,降低血浆BNP,对治疗慢性心力衰竭效果较好。 Objective To observe plasma brain natriuretic peptide(BNP) level and the change trend of cardiac function by using atorvastatin for treating chronic heart failure. Methods 82 cases chronic heart failure patients with cardiac function class II - IV were were randomly divided into treatment group (41 cases) and control group (42 cases). Control group treated with conventional anti-heart failure( vasodilators, diuretics and digitalis) therapy, treatment group on the basis of the control group were given atorvastatin. Observed the effect, BNP level before and after treatment and change of heart function. Results The total effective rate of treatment group(65.9% ) was higher than that of control group(42.9% ) ,the difference was statisti- cally significant ( P 〈 0.05 ). After treatment, in two groups, the left ventricular ejection fraction (LVEF) level were significantly improved, BNP significantly decreased, and LVEF level of treatment group was higher than that of control group, BNP was lower than that of control group,there were significant difference(P 〈 0.05). BNP concentration with LVEF had a negative correlation(r = - 0. 167,P 〈 0.01 ). Conclusion Atorvastatin can improve the heart function of patient with chronic heart failure, increase LVEF, and lower plasma BNP, the treatment of chronic heart failure has a good effect.
