摘要
目的:探讨神经导航系统(BrainLAB Kolibri navigation system)在脑深部肿瘤手术中的应用及其优越性。方法:17例脑内深部肿瘤患者应用神经导航系统辅助下实施显微手术。术前将患者影像学信息导入神经导航系统进行导航计划,并依据肿瘤部位应用神经导航系统设计个体化的手术切口及入路。术中对手术入路、肿瘤及其周围重要解剖结构准确定位,判断病变切除程度。结果:所有病例均一次性直接找到病灶并有效切除。其中全切15例,次全切2例。所有病例临床症状均得到改善或消失。结论:在脑深部肿瘤手术中应用神经导航系统能准确、快速定位肿瘤,清晰辨别周围重要解剖结构,将手术过程中对正常脑组织的损伤最小化,提高手术疗效,减少手术并发症的发生。
Objective:To explore the application and superiority of BrainLAB Kolibri navigation system in the re- section of tumors in deep brain. Methods : All 17 cases with tumors in deep brain underwent microsurgery assisted with neuronavigation system. The radiological information was uploaded into the neuronavigation system to perform navigation plan. The incision and operative approach were designed by neuronavigation system according to the location of tumors. The operative approach, tumor and important structures around the tumors were orientated accurately by neuronavigation system during microsurgery as well as the resection rate were judged precisely. Results: The tumors were found directly and removed properly in all cases. The tumors were totally removed in 15 patients and snbtotally removed in 2 patients. 1 patient got paralysis and aphasia transiently post operation, the symptoms alleviated 2 weeks later. 1 patient received ~/- knife therapy and there was no neurological deficit 3 months after microsurgery. The clini- cal symptoms in all patients were improved or disappeared. Conclusion:The application of neuronavigation - assisted microsurgery to the tumors in deep brain is helpful to accurate localization of the tumors and important structures a- round the tumors. The curative effect can be enhanced, the damage of normal brain and the operative complications can be reduced by the neuronavigation- assisted mierosurgery in the patients with tumors in deep brain.
