摘要 目的:探讨川崎病合并冠状动脉损害发生的高危因素。方法:对1070例川崎病患儿临床资料进行回顾性分析,将川崎病患儿分为冠状动脉损害(CAL)组和无冠状动脉损害(NCAL)组,筛选出川崎病合并冠状动脉损害的相关因素,将筛选的变量行Logistic回归分析。结果:CAL组和NCAL组间性别、年龄、丙种球蛋白治疗时间、发热时间、白细胞、血小板、C反应蛋白(CRP)差异有统计学意义(P<0.05)。Logistic回归分析显示,血小板和CRP升高以及年龄偏大的男孩是川崎病合并冠状动脉损害的危险因素,而丙种球蛋白治疗时间和发热时间延长、白细胞计数升高是保护因素。结论:年龄偏大的男孩,血小板增高,高CRP水平的患儿是川崎病合并冠状动脉损害的高危因素。 Objective: To explore the high risk factors of coronary artery lesions (CALs) caused by Kawasaki disease (KD). Methods: Clinical data of 1 070 patients with KD were collected and retrospectively reviewed. Children with KD were divided into coronary artery lesion (CAL) group and non CAL group (NCAL). The related factors associated with coronary artery lesions were analyzed by Chi-square test,then the risk factors were analyzed by logistic regression. Results: Chi-square test showed that gender, age, gamma globulin treatment time, duration of fever, white blood cell (wBC), platelet (PLT) and C-reactive protein (CRP) were significantly different between CAL group and NCAL group (P 〈 0.05). The logistic regression analysis identified that high level of CRP and PLT and elder age were risk factors in patients with KD and CALs. The long-lasting of gamma globulin treatment time, duration of fever and increased WBC counts were protective factors of CALs. Conclusion: Elder age, male gender, increased PLT and high level of CRP are high risk factors of CALs caused by Kawasaki disease.
