青少年腰椎间盘突出症CT影像观察分析

Observation and analysis of CT image in adolescent with lumbar disc herniation

目的分析青少年腰椎间盘突出症的临床特点及其发生因素,总结其CT影像特点,为临床诊断提供参考。方法分析本科室所收集的42例青少年腰椎间盘突出症病例的临床及影像学资料,分析突出发生的部位及突出的类型,总结分析其特点。结果共收集青少年腰椎间盘突出症病例42例,其中L4～L5发生突出者最多,占71.4%;突出类型分布不均匀,其中以中央型为主,占76.2%;伴随征以侧隐窝狭窄和神经根肿胀为主,增生、"真空"征、黄韧带肥厚、椎体肥大征少见。结论 CT检查可作为诊断腰椎间盘突出症首选方法,值得推广和应用。

Objective To analyze the clinical features and occurred factors in adolescent lumbar disc herniation, summarize the CT image features, and provide a reference for clinical diagnosis. Methods We analyzed the clinical and radiologic da- ta in 42 cases collected in our department, summarized and analyzed its features. Results There were 42 adolescent cases with lumbar disc herniation and a total of 53 discs, of whom L4-L5 disc patietents were the most, accounting for 71.4%; the disc type of uneven distribution, which mainly to central, accounting for 76.2%; accompanied by imposing lateral recess stenosis and nerve root swelling mainly. Hyperplasia, the ＂vacuum＂ sign, yellow ligament hypertrophy, vertebral hypertro- phy were rare. Conclusion There are some differences in CT image between the adolescent and adult. CT scan can be used as the preferred method for diagnosis of lumbar disc herniation, worthy of promotion and application.
