超声斑点追踪技术评价下壁心肌梗死合并右室心肌梗死患者右心室收缩功能 被引量:3

Speckle tracking imaging assessment of right ventricular systolic function in patients with right ventricular myocardial infarction complicating inferior wall myocardial infarction

摘要 目的应用超声斑点跟踪技术评价下后壁心肌梗死合并右室心肌梗死患者右室收缩功能。方法选择右冠轻微病变(右冠狭窄<70%)患者(A组)20例,下后壁心肌梗死合并右室心肌梗死患者(B组)20例,对照组(NC组)为体检健康者30例。应用STI测得右心室游离壁三尖瓣环、室间隔瓣环收缩期峰值位移及三尖瓣环连线中点收缩期峰值位移、右心室纵向缩短率。右室射血分数由实时三维超声心动图测得。结果与NC组比较,A、B组T1、T2、Tm、Tm%、RVEF均明显减低(P<0.05);B组较A组T1、T2、Tm、Tm%、RVEF亦明显减低(P<0.05);各三尖瓣环位移参数与RVEF均显著相关(P均<0.05)。结论 STI可以评价右室心肌梗死患者的右心室收缩功能。 Objective To observe the fight ventricular systolic function in patients with right ventricuiar myocardial infarction in patients complicating inferior wall myocardial infarction with speckle tracking imaging technique. Methods Twenty pa- tients witn stenosis of right coronany atrery〈70%(group A);20 patients with right ventricular myocardial infarction complicat- ing inferior wall myocardial infarction (group B);30 healthy subjects (group NC).The right ventricular free wall, the interven- trcular septum, the midpoint of tricuspid annulus and the right ventricular longitudinal shortening were calculated by using STI,right ventricular ejction were calculated by three dimensional echocardiography. Results The T1,T2,Tm,Tm% ,RVEF were significantly lower in group A and group B than group NC (P 〈 0.05);The T1 ,T2,Tm,Tm% ,RVEF were also significant ly lower in group B than group A (P 〈0.05);The parameters of TAD correlated positively with RVEF (P 〈 0.05). Conclusion The right ventricular systolic function in patients with right ventricular myocardial infarction can be assessed with STI.
