高流量血液滤过的临床护理观察

Clinical nursing observation of high--flow hemofiltration

目的：通过80例脓毒症及多器官功能障碍综合征患者应用连续性血液净化（CBP）治疗的观察和护理，探索治疗过程中常出现的并发症及护理对策。方法：对照组患者进行传统的连续性静脉静脉血液滤过（CVVH），研究组患者进行高流量血液滤过（HVHF），从护理角度对两组患者治疗的效果，以及维持治疗的安全性和连续性，从患者生命体征、滤器使用寿命、避免发生并发症等方面进行重点观察。结果：两组患者滤过后DBP、SBP、HR均有明显改善，研究组患者滤过后的改善程度比对照组更加明显（P〈0．05）。在体温变化、止凝血情况、滤器使用寿命方面，有差异，但无统计学意义（P〉O．05）。结论：HVHF具有较好的临床效果，通过加强护理管理，严密的监测和有预见性的护理，不管是传统的CVVH治疗还是HVHF，均能最大限度减少的并发症，使治疗顺利完成。

Objective：With 80 cases of sepsis and multiple organ dysfunction syndrome patients application continuity blood purification （CBP） the observa- tion and nursing treatment, explore treatment process middling complications and nursing countermeasures. Methods：Patients with the traditional continuity vein vein hemofiltration （CWH）, the team with high flow hemofiltration （HVHF）, from the Angle of nursing two groups were the effects of the treatment, and maintain the security of the treatment and continuity, from patient vital signs, the service life, avoid complications for the observation. Result.Two groups of pa- tients after filter DBP, SBP, HR will be improved obviously, the team with the improvement of the filter degree after more obvious than the control group （P〈 0.05）. In the temperature changes,check blood coagulation,the service life,there are differences,but this was not statistically significant （P〉0. 05）. Conclusion~ HVHF has good clinical effect, by strengthening the nursing management, strict monitoring and prospective care, whether the traditional CWH treatment or HVHF, all can minimize complications, make treatment successfully completed.
