综合干预措施对急诊护士创伤后应激障碍的影响

The Effect of Comprehensive Interventions on Post-traumatic Stress Disorder of Emergency Nurses

目的探讨综合干预措施对急诊护士创伤后应激障碍(PTSD)的影响,寻找缓解PTSD的可行应对方法。方法调查广州市三甲医院急诊护士193人,经过PTSD自评量表评分分值在50分以上40人,按照数字随机法分为综合干预措施组(干预组)和常规方法干预组(对照组),每组各20例,3个月后比较两组急诊护士创伤后应激障碍评分变化。结果干预后,干预组各维度评分较干预前明显降低(P<0.05),对照组仅有警觉性增高较干预前明显降低(P<0.05);组间比较,两组主观评定、重现体验、社会功能以及回避症状评分差异明显(P<0.05),警觉性增高评分比较差异不明显(P>0.05)。干预后,干预组积极应对评分明显增高(P<0.05),消极应对评分明显降低(P<0.05)。组间比较,两组积极和消极应对评分均差异明显(P<0.05)。结论综合干预措施可有效缓解急诊护士面临的PTSD问题,使他们以积极心态进行面对。

Objective To investigate the effect of comprehensive interventions on Post-traumatic Stress Disorder（PTSD） of emergency nurses and the alleviation ways of PTSD.Methods 193 emergency nurses of the Triple-A Hospitals in Guangzhou were surveyed.After self-testing,there were 40 persons whose PTSD scores were ≥50.They were randomly divided into two groups： comprehensive interventions group（intervention group） and intervention group with conventional methods（control group）,20 cases in each group.The PTSD of two groups was compared after 3 months.Results After the intervention,each dimension score of intervention group was significantly reduced than before（P〈0.05）.The scores of increased alertness in control group were significantly lower than before（P〈0.05）.The scores of subjective evaluation,reproduction of experiences,social functions and avoidance of symptoms in the two groups were significantly different（P〈0.05）.The difference of scores of increased alertness were not obvious（P〈0.05）.After the interventions,the active scores of intervention group were increased significantly（P〈0.05）,but the negative scores were significantly reduced（P05）.The positive and negative scores of two groups were significantly different（P〈0.05）.Conclusion The comprehensive interventions can effectively alleviate the problems of PTSD the emergency nurses suffered from to make them have a positive attitude.
