组织细胞坏死性淋巴结炎中CD123及MPO的表达及意义

The expression and significance of CD123 and MPO in histiocytic necrotizing lymphadenitis

目的探讨CD123和MPO在组织细胞坏死性淋巴结炎(histiocytic necrotizing lymphadenitis,HNL)中的表达及在诊断和鉴别诊断中的价值。方法对39例HNL进行常规形态学观察和免疫组化标记,12例淋巴组织反应性增生性病变和12例弥漫性大B细胞淋巴瘤作为对照。结果 CD123在HNL中的阳性率为89.7%,淋巴组织反应性增生性病变中的阳性率为33.3%(P<0.01),弥漫性大B细胞性淋巴瘤中的阳性率为16.7%(P<0.01),其CD123敏感性为90%,特异性为75%;MPO的敏感性为82%,特异性为88%,两者联合应用,其敏感性为79%,特异性为96%。结论 CD123和MPO在HNL中均具有较高的阳性率,其敏感性和特异性均较好,具有很好的诊断和鉴别诊断价值,是诊断HNL较为理想的免疫标记物。

Purpose To investigate the expression of CD123 and MPO in histiocytic necrotizing lymphadenitis （HNL） and the sensi- tivity and specificity of CD123 in the identifying HNL from other diseases. Methods Immunohistochemical staining for CD123 and MPO was carried out on a series of 39 HNL, 14 reactive lymphoid hyperplasia （RLH） and 12 diffuse large B-cell lymphoma （DL- BCL）. Results The expression rate of CD123 was89.7% inHNL, 33.3% inRLH （P〈0. O1） andl6.7% inDLBCL（P〈0.01）. The sensitivity and speicificity of CD123 was 90% and 75% , and MPO was 82% and 88% , respectively. The sensitivity and specifici- ty of the combination of CD123 and MPO was 79% and 96%. Conclusion CD123 and MPO have a high sensitivity and specificity in the diagnosis and differential diagnosis of HNL, both of them are useful immunohisitochemical markers for HNL.
