胶原海绵在慢性下肢溃疡中应用临床疗效观察

Clinical observation of effects on wound healing with collagen sponge application on patients with chronic leg ulcers

目的评价胶原海绵促进下肢慢性溃疡愈合的临床疗效及相关作用机制。方法将40例下肢慢性溃疡患者分为治疗组和对照组,每组各20例。治疗组给予以胶原海绵覆盖创面,对照组给予以凡士林纱布覆盖创面,疗程为4周。分别于治疗后2周、4周采用照相法计算创面愈合率;治疗后1、2、4周评价疮面肉芽积分。结果治疗组创面愈合率治疗后4周高于对照组,差异有统计学意义;疮面肉芽积分在2周与4周时达到统计学差异(P<0.05或0.01)。结论胶原海绵可以促进慢性下肢溃疡肉芽生长,提高慢性疮面愈合率。

Objective To evaluate the clinical effects of collagen sponge application on chronic leg ulcer and to explore the possible relationship with improving the growth of granulations of wound. Methods Forty patients of chronic leg ulcers were included in the research, half for experiment group with the local administration of collagen sponge and the other half for the control with the local administration of petrolatum gauze. The appearances of granulation tissues were evaluated as scores for the dates of 1 -week, 2-week and 4-week. The wound surface healing rates evaluated by electronic wound medical record system were recorded for the dates of 2-week and 4-week. Results The rates of wound surface healing and the appearances of granulation tissues were higher with experimental group than those of control group at dates of 4-week and 2-week together with 4-week respectively with statistic differences （ P 〈 0.05 or 0.01 ）. Conclusion Collagen sponge is able to improve wound healing which may be one way of increasing the growth of wound granulations.
