临床病例（理）讨论第120例——发热伴双肺多发结节及全组鼻窦病变

摘要 临床资料 患者女，66岁，教师。因“咳嗽、发热6月余，加重10d”于2011年8月27日入院。患者2011年2月无明显诱因出现间歇性发热，发作无规律，体温约38～40℃，可自行恢复正常。伴阵发性咳嗽，咳少量白色黏痰；鼻塞，流脓涕，咽痛，声音嘶哑。先后多次在外院住院治疗，给予抗感染治疗，疗效欠佳。2011年6月10日行肺部CT检查无明显异常（图1）。鼻咽部＋鼻窦平扫CT示全组鼻窦炎症。电子鼻咽镜示喉炎，真菌性可能性大。腹部B超示腹膜后低回声结节，考虑淋巴结肿大可能。诊断为：“真菌性喉炎可能性大；慢性鼻窦炎等”。
