原发性腹膜后肿瘤MR-DWI诊断价值的探讨

Value of MR-DWI in diagnosis of retroperitoneal tumor

为了探讨常规MR结合DWI及其ADC值对腹膜后肿瘤的诊断价值,回顾性分析18例腹膜后肿瘤患者的临床资料,结合手术病理结果,分析肿瘤的常规MR表现、DWI信号特点及其ADC值等影像学特征。18例腹膜后肿瘤中,良性7例,恶性11例。MR定位诊断符合率为88.89%(16/18),良恶性诊断符合率为77.78%(14/18),组织类型符合率66.67%(12/18)。本组7例良性腹膜后肿瘤的ADC值为(1.88±0.58)×10-3 mm/s,11例恶性腹膜后肿瘤的ADC值为(1.06±0.45)×10-3 mm/s,差异无统计学意义,P<0.05。初步研究显示,综合分析常规MR、DWI及ADC值等影像表现不仅对腹膜后肿瘤的检出及定位有较为可靠的价值,而且有助于肿瘤的良恶性判定及组织类型诊断。

OBJECTIVE： The objective of this study was to investigate the value of routine-MR DWI and ADC values in the diagnosis of retroperitoneal tumor. The imaging findings of routine-MR and DWI of 18 retroperitoneal tumor were retrospectively analyzied and correlated with surgical and pathologic findings. The 18 retroperitoneai tumor including 7 ca- ses of benignancy, 11 cases of malignance. The correct rate of MR localization was 88.89%（16/18）. The accurate rate of MR in the prediction of benignancy or malignance was 77.78%（14/18）. The accurate rate of MR in the prediction pathol- ogy organization type of retroperitoneal tumor was 66.67%（12/18）. The ADC values of benignancy and malignance were （1.88±0.58）×10.3 mm/s and （1.06±0.45）×10 3 mm/s, and the ADC values of benignancy and malignance showed significant differences （P〈0.05）. In conclusion, there are certain reference values in the localization and qualitative of ret- roperitoneal tumor by generalized analysis the imaging findings of MR,DWI and ADC values.
