医护人员对重性精神病患者参与治疗的态度和实施现状

Attitude and implementation of health care workers on patient involvement in treatment decision making.

目的探讨精神科医护人员对重性精神病患者参与治疗的态度和实施现状。方法将宝鸡市康复医院内的120名医护人员作为调查对象,应用自制的调查问卷对医护人员关于重性精神病患者参与治疗的态度和实施进行调查。应用统计学方法分析医院医护人员对重性精神病患者参与治疗的态度和实施现状。结果 69.3%的调查对象对"患者参与治疗"的概念熟悉,有7.0%的调查对象对"充权"概念熟悉。对患者参与诊断报告、药物使用决策、治疗过程的支持态度分别占调查对象的56.1%、69.3%、41.2%;在具体实施上支持的分别占调查对象的43.9%、24.6%、24.6%。医护人员对重性精神病患者参与治疗的实施明显低于态度(P<0.01)。医护人员对患者参与治疗的知识、态度和实施与其受教育程度、年龄及工作年限呈负相关(P<0.05)。结论精神科医护人员对重性精神病患者参与治疗支持的态度较高,但在实施上力度不足。医护人员对患者参与治疗的重视程度与其受教育程度、年龄及工作年限有关。

Objective To explore the attitude and implementation of health care workers on patient involvement in treatment decision making. Methods One hundred and twenty health care workers in Rehabilitation Hospital of Baoji City were selected as research objects. Self-made questionnaire was used to investigate the attitude and implementation on patient involvement in treatment decision making, with statistical analysis performed. Results 69.3% of the research objects were familiar with the concept of patient involvement in treatment decision making, and 7.0% were familiar with the concept of ＂empowerment＂. Subjects held supportive attitude on patient involvement in the diagnostic report, drug use decision making, and treatment process accounted for 56.1%, 69.3%, 41.2%, and those were supportive in the specific implementation accounted for 43.9%, 24.6%, 24.6%. The implementation rate were significantly lower than the rate of supportive attitude （P〈0.01）. The knowledge, attitudes, and the implementation of the clinical staffs were negatively correlated with their educational level, age and work experience （P〈0.05）. Conclusion Psychiatric health care workers hold highly positive attitude towards patient involvement in treatment decision making, but the implementation is insufficient.
