摘要
目的探讨白细胞介素-1α(IL-1α)-889基因多态性与慢性牙周炎(chronic periodontitis,CP)易感性的相关性。方法系统检索中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、PUBMED、MEDLINE和EMBASE数据库。对纳入研究的文献采用Newcastle-Ottawa量表进行质量评价。Meta分析采用RevMan 5.1.2和Stata 11.0进行。合并效应采用比值比(OR)和95%可信区间(95%CI)进行评价。发表偏倚通过绘制漏斗图直观判断和Egger回归法、Begg秩相关法进行量化检测。敏感性分析为剔除不符合HW平衡的文献后重新进行Meta分析。结果 19个研究纳入Meta分析,共有慢性牙周炎患者1 949例,健康对照1 455例。IL-1α-889基因多态性与CP易感性相关(等位基因模型:OR=1.41,95%CI=1.17~1.70,P=0.000 3;加性模型:OR=1.58,95%CI=1.20~2.10,P=0.001;显性遗传模型基因型:OR=1.61,95%CI=1.27~2.05,P<0.000 1;隐性遗传模型基因型:OR=1.39,95%CI=1.07~1.79,P=0.01)。结论IL-1α-889多态性与慢性牙周炎易感性相关联,T等位基因携带者遗传易感性上升。
Objective To study the correlation between IL-1α-889 polymorphism and susceptibility to chronic periodontitis (CP). Methods Papers on correlation between IL-1α-889 polymorphism and suscepti- bility to CP were systematically retrieved from CBM, CNKI, PUBMED, MEDLINE and EMBASE. Academic level of papers included in this study was assessed according to the Newcastle-Ottawa Scale (NOS). Meta-anal- ysis of the correlation between IL-1α-889 polymorphism and susceptibility to CP was carried out using the Re- view Manager ( Version 5.1.2) and Stata ( Version 11.0), respectively. Pooled odds ratio (OR) and 95 % CI for the correlation between IL-1α-889 polymorphisms and susceptibility to CP were evaluated. Publication bias in the papers was tested by funnel plot, Egger' s regression test and Begg' s test, respectively. Sensitivity analy- sis was performed for the exclusion of papers not conforming to HWE, followed by a repeat Meta-analysis. Results Nineteen papers covering 1 949 cases of chronic periodontitis and 1 455 controls were included in the meta-analysis. Our study showed that IL-1α-889 polymorphism was correlated with the susceptibility to CP in T allele model (OR = 1.41, 95% CI = 1.17 - 1.70, P =0. 000 3) , T/T model (OR = 1.58, 95% CI = 1.20 - 2. 10, P = 0. 001 ) , dominant gene model ( OR = 1.61, 95% CI = 1.27 - 2.05, P 〈 0. 000 1 ) , and recessive gene model (OR =1.39, 95%C1=1.07 -1.79, P=0.01). Conclusion IL-1α-889 polymorphism is corre- lated with the susceptibility to CP. The susceptibility to CP increases in T allele carriers.
