影响脊髓型颈椎病椎板成形术的预后因素分析

摘要 目的通过对比分析方法对影响脊髓型颈椎病椎板成形术预后的相关因素进行分析研究。方法本研究共24例患者在2001年6月至2010年6月间行颈椎板成形术,术后定期随访,随访时间≥12个月。对患者年龄、手术节段数量、脊髓压迫比率、脊髓压迫节段数量、脊髓压迫节段水平、T2像上脊髓局部边缘高信号、术前JOA评分等预后因素分别进行分析,了解其与恢复率之间的关系。结果脊髓压迫比率与恢复率呈正相关(r=0.754,P=0.000),脊髓压迫节段数量、脊髓压迫节段水平、他相上高信号节段数量及边缘组织图像级别均与恢复率呈负相关(r=-0.548,P=0.013;r=-0.866,P=0.001;r=-0.551,P=0.012;r=-0.497,P=0.026),年龄、手术节段数量、术前JOA评分与恢复率之间无关(P〉0.05)。结论脊髓压迫比率、脊髓压迫节段数量、脊髓压迫节段水平、他相上高信号改变的局部边缘影像等因素在影响椎板成形术后预后方面起重要作用。
