NT值异常与胎儿心脏畸形及染色体异常之间的相关性 被引量:16

摘要 目的探讨胎儿颈项透明层厚度(NT值)异常与胎儿心脏畸形及染色体异常之间的相关性。方法回顾性分析2008年6月至2011年6月2 959例单胎孕妇的临床资料,根据孕周11~14周的NT值将其分为≥3.5 mm组(103例)和<3.5 mm组(2 856例)。NT值≥3.5 mm组或胎儿异常于孕周18~24周内进行超声检测及染色体核型分析。结果 NT值≥3.5 mm组的的胎儿的染色体异常、心脏畸形及染色体异常合并心脏畸形的比率明显高于NT值<3.5 mm组(P<0.01),且NT值<3.5 mm时,心脏畸形及染色体异常与NT值呈负相关性,NT值≥3.5 mm时,心脏畸形及染色体异常与NT值呈正相关性。结论 NT值异常与胎儿心脏畸形及染色体异常之间存在明显的相关性,NT值可为临床作出正确处理提供可靠的依据。 Objective To explore the correlation between abnormal value of nuchal translucency(NT) with cardiac teratosis and abnormality of chromosomes in fetuses.Methods The clinical data of 2 959 pregnant women with single birth in this hospital during June 2008 to June 2011 were retrospectively reviewed and analyzed.They were allocated into NT≥3.5 mm group and NT3.5 mm group according to their value of NT during 11 to 14 weeks of pregnancy.Pregnant women in NT≥3.5 mm group or with abnormal fetuses were examined by ultrasonography and karyotype analysis of chromosomes during 18~24 weeks of pregnancy.Results The proportion of occurrence of abnormal chromosomes,cardiac teratosis and abnormal chromosomes associated with cardiac teratosis in fetuses of NT≥3.5 mm group was significantly higher than those of fetuses in NT3.5 mm group,their difference was statistically significant(P0.01).Furthermore,as fetus with value of NT3.5 mm,the incidence rate of cardiac teratosis and abnormal chromosomes was negatively correlated with the value of NT,when the value of NT≥3.5 mm,the incidence rate of cardiac teratosis and abnormal chromosomes in fetus was positively correlated with the value of NT.Conclusion There is significant correlation between abnormal value of NT with cardiac teratosis and abnormal chromosomes in fetus,hence the value of NT in fetus can be applied as a confident evidence for correct management of pregnancy.
