CD34和α-SMA在乳腺良恶性肿瘤间质中的表达与意义 被引量:4

摘要 目的探讨CD34和α-平滑肌肌动蛋白(α-SMA)在乳腺良恶性肿瘤间质中的表达情况及其意义。方法收集2008年1月-2011年3月在新疆医科大学附属肿瘤医院住院行手术治疗并术后经病理科诊断为乳腺导管上皮普通型增生(50例)、乳腺导管上皮非典型增生(50例)、乳腺导管原位癌(50例)及乳腺浸润性导管癌(50例)标本。应用免疫组织化学PV-9000法检测乳腺间质细胞中CD34和α-SMA的表达。结果 CD34在乳腺导管上皮普通型增生、乳腺导管上皮非典型增生、乳腺导管原位癌及乳腺浸润性导管癌间质中的表达率分别为84%、78%、48%及32%,差异有统计学意义(χ2=40.430,P=0.000),乳腺浸润性导管癌CD34阳性表达率低于乳腺导管上皮普通型增生、乳腺上皮非典型增生及乳腺导管原位癌。α-SMA在乳腺导管上皮普通型增生、乳腺导管上皮非典型增生、乳腺导管原位癌及乳腺浸润性导管癌间质中的表达率分别为28%、46%、62%及86%,差异有统计学意义(χ2=56.772,P=0.000),乳腺浸润性导管癌α-SMA阳性表达率高于乳腺导管上皮普通型增生、乳腺上皮非典型增生及乳腺导管原位癌。结论富含α-SMA阳性表达而缺少CD34阳性表达的乳腺间质细胞更加有利于肿瘤的发生。 Objective To explore clinicopathologic expression of CD34 and α-SMA in stromal cells of benign and malignant breast tumor. Methods Two hundred cases of breast carcinoma including usual ductal hy- perplasia (n =50), atypical ductal hyperplasia (n =50), ductal carcinoma in situ (n =50), invasive ductal carcinoma (n =50) were collected, the expression of CD34 and α-SMA were detected by immunohisto chemistry in stromal cells. Results The positive rates of CD34 in usual ductal hyperplasia, atypical ductal hy perplasia, ductal carcinoma in situ and invasive ductal carcinoma were 84%, 78%, 48% and 32% with statistical significance (X^2=40. 430, P =0. 000). The positive rate of invasive ductal carcinoma was lower than the ones ofusual ductal hyperplasia, atypical ductal hyperplasia, ductal carcinoma in situ. The positive rates of a-SMA in usual ductal hyperpla noma were 28 %, 46%, sia, atypical ductal hyperplasia, ductal carcinoma in situ and invasive ductal carci 62% and 86%, with statistical significance (X^2=56. 772, P =0. 000). The posi tive rate of invasive ductal carcinoma was higher than the ones of usual ductal hyperplasia, atypical ductal hyperplasia, ductal carcinoma in situ. Conclusion The breast stromal cells, which are rich of expression of a-SMA and lack of expression of CD34 can easily lead to tumorigenesis.
