摘要
[目的]探讨2010年滨州市手足口病重症病例发生的影响因素。[方法]在2010年滨州市手足口病病例中,选择70例重症病例及按照1∶2的比例选择的140例普通病例进行个案调查,并采用非条件Logistic回归分析影响手足口病重症病例发生的因素。[结果]多因素非条件Logistic回归分析结果,手足口病重症病例的发生有4个危险因素和2个保护因素,危险因素为就诊前最高体温(OR=4.676)、嗜睡(OR=11.336)、心律失常(OR=7.012)、EV71型感染(OR=2.691),保护性因素为年龄较大(OR=0.450)和初诊到县级以上医院(OR=0.001)。[结论]及时到具有救治能力的医院就诊有助于减少手足口病重症病例的发生。
[Objective]To explore the influencing factors of serve Hand-foot-mouth disease（HFMD） in Binzhou city, 2010. [Methods]A case-control study involving interviews with 70 newly diagnosed mild HFMD cases and selected 140 ca- ses of clinical diagnosis in patients with HFMD of controls according to the proportion of 1 ： 2 matching for age and sex. Statistical analyses of single-factor and multivariate conditional logistic regression were performed to investigate the risk factors of HFMD.[Results]By using single-factor analysis, 8 exposure factors were identified as aetiologieal factors for HFMD. Multivariate conditional logistic regression analysis showed 4 main risk factors and 2 protective factors for HFMD. The highest temperature before treatment（ OR =4. 676） ,drowsiness（ OR = 11. 336） ,arrhythmia（ OR = 7. 012） and EV71 infection（ OR =2. 691） were main risk factors of severe HFMD. Older ages（ OR =0. 450） and treatment above the county level hospitals（ OR= 0. 001） were protective factor of severe HFMD. [Conclusion]Timely treatment at hospi- tals with rescue and treatment capabilitiesl helps to reduce the occurrence of severe cases of hand, foot, and mouth disease.
