摘要
目的:通过经颅多普勒超声连续观测针刺左侧大包穴在不同时间对健康男性青年大脑后动脉(PCA)血流动力学、心率及血压的变化情况,探讨针刺左侧大包穴对大脑后动脉(PCA)血流动力学、心率及血压影响与时间的相关性。方法:随机选取健康男性青年8名,平均年龄(24±2)岁。安静状态下,针刺受试者的左侧大包穴,用经颅多普勒超声仪(TCD)探头于双侧颞窗探及大脑后动脉(PCA),在针刺前10min、进针时、行针时(留针15min)、出针时(留针30min)及出针后10min 5个时间点,检测大脑后动脉血流动力学变化,记录收缩期最大血流速度(Vs)、舒张期最大血流速度(Vd)、平均血流速度(Vm)、搏动指数(PI)及阻抗指数(RI)各参数值,并在每次检测后立即采用电子血压计测量并记录血压及心率数据,进行统计学分析。结果:针刺大包穴,在进针时、行针时、出针时、出针后10min的双侧收缩期最大血流速度(Vs)、舒张期最大血流速度(Vd)、阻抗指数(RI)、搏动指数(PI)与针刺前10min相比均无统计学意义(P>0.05);行针时左侧平均血流速度(Vm)与针刺前10min相比有显著统计学意义(P<0.05),右侧Vm无统计学意义;心率在进针时与进针前相比P<0.05,出针后10min与进针前相比P=0.05,均有统计学意义,其余时间点心率数据无统计学意义;本实验中,血压(收缩压、舒张压)在进针时、行针时、出针时及出针后10min与针刺前相比,均无显著差异(P>0.05)。结论 (:1)针刺左侧大包穴在针刺留针15min时能显著增加左侧大脑后动脉(PCA)的平均血流速度(Vm)。(2)针刺左侧大包穴在进针时及出针后10min,心率在生理范围内减慢。(3)针刺左侧大包穴对血压无显著影响。
Objective : We observed the Trans Cranial Doppler Ultrasonography ( TCD ) of Posterior Cerebral Artery (PCA)to study the therapeutic mechanism and the effect of the Dahao Point with acupuncture. Methods:Eight healthy male youth were randomly selected, average age was (24 +2 )year's old. Below quiet condition, with TCD probe in bilateral temporal window and posterior cerebral artery( PCA ), acupuncture those tested left acupuncture, point, five time points included 10 minutes before insertion of the needle, manipulating the needle, retaining the needle ( 15 minutes ), retaining the needle ( 30 minutes ), 10 minutes after withdrawing the needle, test the change of hemodynamie of PCA, record the systolic maximum flow velocity ( Vs ), diastolic maximmn flow velocity ( Vd ), average flow velocity ( Vm ), beating index (PI) and impedance index (R1)parameters, and in each test measure their blood pressure and heart rate immediately, record the date, conduct statistical analysis. Results : Acupuncture in SP 21, when insertion of the needle, nmnipulating the needle, withdrawing the needle, and 10 minutes after withdrawing the needle, the bilateral systolic maximum flow veloeity ( Vs ), diastolic maximum flow velocity ( Vd ), impedance index ( HI ) and beating index ( PI ) compared to those 10 minutes before insertion of the needle, there were no statistically significant differences (P〉0.05) .When manipulating the needle, the left Vm compared to that 10 minutes before insertion of the needle increased significantly (P〈0.05), and the right Vrn was riot statistically significant ; the heart rate of insertion of the needle compared to that 10 minutes before insertion of the needle, P〈O.05 ; lO minutes after withdrawing the needle compared to that 10 minutes before insertion of the needle, P=0.05, which was statistically significant, the rest of the time points of heart rate were not significant. In this study, blood pressure (systolic blood pressure and diastolic blood pressure )when the needling in every time points compared to that 10 minutes before insertion of the needle, there was no significant difference (P〉0.05) .Conclusion : ( 1 ) Acupuncture could raise the Vm of PCA when Dabao Point was treated. ( 2 ) Acupuncture could slow heart rate within the physiological scope when insertion of the needle and 10 minutes after withdrawing the needle. ( 3 ) Acupuncture makes no significant effect on blood pressure.
