舒肝活血化瘀合剂对于中年后妇女高脂血症之临床疗效评估

Clinical Evaluation of Shugan-Huoxue-Huayu Mixtures for Middle-Aged Women's Hyperlipidemia

目的：本研究主要探讨中医辨证治疗中老年人高脂血症的临床治疗疗效。方法：本研究共收集25例高脂血症中年以上妇女,其年龄43-76岁,平均年龄（61.36±9.69）岁;发病病程1.5-10年,平均病程为（5.16±2.74）年。单纯胆固醇增高者25例,单纯甘油三酯增高者24例,胆固醇与甘油三酯均增高者24例,高密度脂蛋白低者12例;有家族史者有12例,经过西医治疗过者有12例,12例有中度脂肪肝。结果：经过舒肝活血化瘀合剂4个月的治疗后血中胆固醇治疗前为（239.3±34.1）mg/dL,治疗后为（204.2±29.7）mg/dL,呈显著的统计学上差异（P〈0.001）;三酸甘油脂治疗前为（168±5.96）mg/dL,治疗后（124.2±33.1）mg/dL,（P〈0.001）呈明显差异。血中HDL-C治疗前（60.2±15.5）mg/dL,治疗后（68.8±9.9）mg/dL,其P〈0.05;LDL-C治疗前（148.1±33.5）mg/dL,治疗后（123.9±18.5）mg/dL,其P〈0.001均呈明显差异。舒肝活血化瘀合剂降血脂之作用机转可能与其能使血中HDL-C升高,而使外围血脂能顺利的经血流循环带回肝脏代谢,因而达到降低血脂的功效有关。其中12例并有中度脂肪肝患者,治疗后均转为轻度脂肪肝。在药物不良反应观察方面,在整个服药过程中,除少数病例（3例）出现肠鸣腹胀气等症状外,未见其他不良反应;且肝、肾功能均未发现异常。结论：高脂血症的中医临床治疗原则是以活血化瘀、消食化痰、舒肝理气、泻下清热、淡渗利水为主,本研究之舒肝活血化瘀合剂对于中年后妇女高脂血症的治疗有很好的疗效。其确切的药理作用机转能有待作进一步的探讨。

Objective ： This study was aimed to investigate the clinical efficacy of SHHM on the hyperlipidemia in middle-aged women patients. Methods：We collected 25 middle-aged hyperlipidemia patients. They were 43-76 years old, mean age （ 61.36 ±9.69 ）years old, 1.5-10 Morbidity course,25 hypercholesterol patients,24 hypertriglyeerol patients,24 high TC±TG, 12 low HDL level, 12 family history patients, 12 drag-treated patients and 12 Mid-fatty liver. Results ： The 25 patients after SHHM treatment for 4 months, the levels of TC, TG and LDL level were decreased significantly （P〈0.05-0.001）. The level of HDL was increased significantly （P〈0.001）. The hypolipidemic mechanism of SHHM may be related to the increase in the HDL level. The 12 fatty liver patients were improved after SHHM treatment. There were only 3 patients showed aabdominal rumbling and inflation. Conclusion ： The major principles of clinical treatment of hyperlipidemia are promoting blood circulation and removing extravasted blood, digestion and phlegm reduction, extends the liver to regulate the flow of vital energy, flows swiftly and alleviates fever, and Infiltrates the favorable water pale. SHHM possesses efficacy on hyperlipidemia in mid-life women. The hypolipidemic mechanisms of SHHM need to investigate in the future.
