动脉瘤大小对前交通动脉瘤夹闭术患者预后的影响 被引量:3

摘要 目的探讨前交通动脉瘤大小和临床预后的关系。方法回顾性分析我院神经外科2006年1月1日至2011年1月1日期间由同一组医师开颅手术治疗的209例前交通动脉瘤患者的临床资料、影像学及预后,按动脉瘤大小,分为小型组(<1cm)、中型组(1~1.5cm)、大型组(1.5~2.5cm)、巨大型组(≥2.5cm),预后用格拉斯哥评分进行比较。结果 209例手术治疗的前交通动脉瘤患者中预后良好者134例,其中包括9例小型动脉瘤,62例中型动脉瘤,48例大型动脉瘤和15例巨大型动脉瘤;不同动脉瘤大小的患者预后良好率、脑梗率、脑血管痉挛率差异均有统计学意义(P<0.05),中型大小的动脉瘤(1~1.5cm)预后良好率较其他组高,中型动脉瘤组的患者术后发生脑梗死的率较其他组低,而发生脑血管痉挛率较其他组高。结论动脉瘤大小是影响前交通动脉瘤夹闭术患者预后的危险因素;中型大小的动脉瘤(1~1.5cm)预后较其他组好。
