COPD合并肺性脑病无创正压通气治疗的护理

The nursing of noninvasive positive pressure ventilation on the treatment of chronic obstructive pulmonary disease combine with pulmonary encephalopathy

目的:观察慢性阻塞性肺疾病(COPD)合并肺性脑病无创正压通气(NIPPV)治疗及护理的疗效。方法:对68例COPD合并肺性脑病患者在常规治疗基础上给予双水平无创正压通气(BiPAP)治疗,观察治疗前后患者的临床症状、动脉血气指标的变化。结果:39例6 h内意识转为清楚,12例6～12 h意识转为清楚,7例13～24 h意识转为清楚。58例治疗后动脉血气指标较治疗前改善(P<0.05),10例疗效不佳改行有创机械通气。结论:NIPPV治疗COPD并肺性脑病,能快速缓解症状,纠正呼吸衰竭,同时配合有效的护理措施,疗效肯定,值得临床推广应用。

Objective：To study the efficacy of noninvasive positive pressure ventilation（NIPPV）in the treatment of chronicobstructive pulmonary disease（COPD）combine with pulmonary encephalopathy.and cooperating the nursing care.Methods：68 patients diagnosed as COPD combine with pulmonary encephalopathy were given conventional therapy and Bi-level positive airway pressure（Bi-PAP）therapy.The clinical symptoms before and after treatment,arterial blood gas change were observed.Results：In 68 patients,there were 39 patients′awareness turning clear within 6h,12 cases within 6-12 h,7 cases within 12-24 h;after treatment,arterial blood gas targets in 58 cases were significantly improved than before treatment（P0.05）.10 cases of poor effect were diverted to the treatment of invasive mechanical ventilation.Conclusion：NIPPV treatment of COPD and pulmonary encephalopathy can quickly re-lieve symptoms,correct respiratory failure,and is effective and worthy of clinical application At the same time cooperating the effective nutsing care.
