支气管肺泡灌洗加局部抗生素治疗支气管扩张伴感染的疗效

Therapeutic effects of bronchoalveolar lavage plus local antibiotic treatment on bronchiectasis accompanied with infection

目的研究支气管肺泡灌洗联合局部抗生素治疗支气管扩张并发感染的有效性。方法采用前瞻式病例对照研究,临床入选124例支气管扩张并发感染患者,按照入院先后顺序编号,根据随机数字方法分成2组,每组62例,分别为对照组(常规治疗)和研究组(支气管肺泡灌洗联合局部抗生素治疗),治疗后比较两组患者的相关临床指标。结果研究组的住院天数和住院总费用均较对照组明显减少(P<0.05)。两组患者治疗后的PaO2、SaO2及PaO2/FiO2均较治疗前明显改善(P<0.05),但治疗后研究组的这些指标较对照组改善更显著(P<0.05)。研究组总有效率为100%,高于对照组的85.48%(P<0.05)。结论支气管肺泡灌洗联合局部抗生素治疗支气管扩张并发感染不仅能够显著提高临床效果,而且节省医疗资源。

Objective To investigate the efficacy of bronchoalveolar lavage combined with local antibiotic treatment on bronchiectasis accompanied with infection. Methods Prospective case control study was carried out, and 124 cases of bronchiectasis accompanied with infection were clinically selected and randomly divided into two groups：control group（ conventional treatment, n = 62 ） and research group（ bronchoalveolar lavage combined with local antibiotic treatment, n = 62 ）. The clinical indexes of the patients in the two groups were compared after the treatment. Results Compared with the indexes of the control group, the hospital day and total cost of hospitalization in the research group decreased significantly（ P 〈 0.05 ）. Compared with the indexes before the treatment,PaO2 ,SaO2 and PaO2/FiO2 of the two groups were significantly improved after the treatment（ P 〈 0.05 ）. But compared with those indexes of the control group, those of the research group were improved more significantly （ P 〈 0.05 ）. The total effective rate of the research group was 100% ,which was higher than that of the control group（85.48% ）. Conclusion Bronchoalveolar lavage combined with local antibiotic treatment on bronchiectasis accompanied with infection can not only significantly improve the clinical outcomes, but also save the medical resources.
