临床药师参与317例细菌感染性病例会诊分析

Clinical pharmacists participating in consultation:analysis of the 317 bacterial infection cases

目的:分析临床药师参与的细菌感染性病例会诊特点及变化趋势,为临床药师进一步开展工作提供参考。方法:对2009–2011年我院临床药师参与会诊的细菌感染性病例进行分析,包括会诊数量、会诊原因、科室分布、病例特点、感染细菌、抗菌药物应用情况、会诊意见接纳程度及会诊效果等。结果:2009–2011年我院临床药师共参与感染性病例会诊317例次,其中2011年会诊占66.6%;会诊病例中无细菌培养及药敏结果的经验性用药会诊以及会诊前仅使用一种抗菌药物且用药时间不超过7天的病例比例逐年增加;会诊采纳率及临床有效率均逐年上升。结论:临床药师参与细菌感染性病例会诊的数量、范围、意见接纳程度及临床效果逐年提高,但仍存在临床经验不足、知识陈旧等问题,尚需进一步提高会诊能力。

Objective： To analyze the characteristic and tendency of bacterial infection cases, and provide reference for clinical pharmacists. Methods： The consultation data of bacterial infection cases which clinical pharmacists participated in were analyzed, including the amount and the characteristic of consultation, the reasons for consultation, department distribution, characteristics of the cases, infectious bacteria, medication history, consultative Suggestion adopted and feedback of patient＇s condition, etc. Results： A total of 317 consultation cases were collected, and the ratio of consultation was 66.6% in 2011. Among these cases, the proportion of no drug sensitivity test and only used one antibacterial for less than 7 days was increased. Meanwhile, the adoption rate of clinical pharmacists＇ advices and clinical efficiency were also increased. Conclusion： Clinical pharmacists made a great progress in consultation, including the amount, the rate of adoption and clinical efficiency. But there were still several disadvantages, such as deficiency of clinical experience, short of updating intelligence, etc. Therefore clinical pharmacists still should continue working hard to improve ability further.
