脑弥漫性轴索损伤的螺旋CT诊断

Spiral CT diagnosis of brain diffuse axonal injure

目的:分析脑弥漫性轴索损伤(diffuse axonal injure,DAI)的螺旋CT表现,探讨螺旋CT对DAI的诊断价值。方法:收集36例经临床证实为DAI病例的CT及临床资料进行回顾性分析。结果:本文36例,其中弥漫性脑肿胀31例,脑室及脑沟、裂、池均受压变小或闭塞;脑实质出血29例,其中胼胝体区出血8例,基底节区5例,脑干4例,小脑半球2例,皮髓质交界区18例,15例为上述两个或多个部位合并受累;蛛膜网膜下腔出血19例,脑室出血6例,硬膜下血肿8例,硬膜外血肿3例,颅骨骨折6例。结论:螺旋CT扫描可为临床诊断DAI提供较直接、可靠的影像学依据,对临床早期诊断和治疗DAI具有重要价值。

Objective To investigate CT value in diagnosing diffuse axonal injure （DM）. Method CT and clinical material were used to analyze retrospectively in 36 cases with DM clinical proved. Resulls Total 36 cases with DAI,31 cases of them with diffuse brain swell- ing showed the smaller or occlusion of brain ventricle,sulcus,fissure and cistern,29 cases were with cerebral hemolThage,8 cases located in corpus callosum,5 cases were in basal ganglia,4 cases were in brain stem,2 cases were in cerebellar hemisphere,18 ease were in cortical medullary junction,15 cases had two or more parts involved. Nineteen cases were with subarachnoid hemorrhage,6 cases were with brain ventricle hemorrhage,8 eases with subdural hematoma,3 cases with epidural hematoma and 6 cases with skull fracture. Conclusion Spiral CT has an important value in early diagnosis and treatment of DAI.
