透光旋切系统治疗大隐静脉曲张337例临床分析

Clinical analysis of transilluminating rotating resecting system in treatment of varicose great saphenous vein: a report of 337 cases

目的探讨透光旋切系统治疗大隐静脉曲张的临床疗效。方法以透光旋切系统治疗下肢大隐静脉曲张患者共337例(386条肢体)。结果 337例手术均获成功。单条肢体手术时间21～54 min,平均(31.2±4.6)min;切口数量3～5个,平均(4.1±0.6)个,住院时间3～15d,平均(4.8±2.5)d。2条患肢术后出现局部皮肤破溃坏死,1条出现深静脉血栓。术后患肢出现皮下淤血154条,血肿11条,肢体局部麻木感112条。337例患者共386条肢体随访6～24个月,平均(15±6.7)月。皮下淤血及血肿消退,肢体局部麻木感消失95条,减轻27条。术前存在色素沉着的肢体,术后均不同程度减轻,沉重感缓解,无静脉曲张复发,小腿外观无明显手术疤痕。结论透光旋切系统治疗大隐静脉曲张安全可行,具有微创、手术时间短、术后恢复快、美容效果好等优点。

Objective To discuss the clinical efficacy of TriVexTM in treatment of varicose great saphenous vein. Methods Three hundred and thirty seven patients with lower extremity varicose great saphenous vein were treated by TriVexTM. Results The operation was performed successfully in all the 337 cases. The operation time per leg was 21 -54 min, （31.2 ±4.6） min averagely. The number of incisions for phlebectomy was 3 -5, （4.1 ±0.6） averagely. The hospital stay was 3 - 15d,average （4.8 ±2.5）d. Postoperative complications included local skin ulceration in 2 legs, deep vein thrombosis in 1 leg, bruising in 154 legs, hematoma in 11 legs, and local paraesthesia in 112 legs. Follow-up checkups were carried out in 337 cases （386 legs） for6 -24 months, average （ 15 ± 6.7） months. Bruising and hematoma disappeared in all the cases. Local paraesthesia vanished in 95 legs and subsided in 27. The preexisting pigmentation decreased in various degrees, and heavy feeling was relieved. No recurrence of varicosis was noted. No prominent scar was found in the calf region. Conclusion TriVexTM used in varicose great saphenous vein removal is safe and effective, which could reduce the trauma, shorten the operating time and has advantages of rapid recovery and cosmetic function.
