大功率绿激光选择性前列腺汽化术与电切术疗效比较 被引量:2

摘要 目的比较研究经尿道绿激光选择性前列腺汽化术(photoselective vaporization of the prostate,PVP)与经尿道前列腺电切术(TURP)的疗效。方法将496例良性前列腺增生(BPH)患者分为两组,255例行PVP术,241例行TURP术,比较两组的手术时间、导尿管留置时间、术后住院天数、手术并发症。随访3个月比较两组最大尿流率(Qmax)、剩余尿量(PVR)、国际前列腺症状评分(IPSS)、生活质量评分(QOL)等指标的变化。结果 PVP组、TURP组的手术时间、导尿管留置时间、术后住院天数分别为(54.9±20.0)min及(45.1±18.0)min、(1.1±0.2)d及(3.4±0.3)d、(3.2±1.1)d及(8.8±1.3)d,组间比较差异有统计学意义(P<0.05)。术后3个月242例PVP患者和226例TURP患者获随访,Qmax、PVR、IPSS、QOL较术前均有改善,差异有统计学意义(P<0.01);组间比较差异无统计学意义(P>0.05)。结论PVP术与TURP术相比,手术安全性更好,疗效相似。 Objective To compare the efficacy between photoselective vaporization of the prostate of green laser with high- power and transurethral resection of the prostate (TURP). Methods Four hundred and ninety-six cases of BPH were divided into two groups, in which 255 cases were given PVP and 241 were given TURP. The operating time, catheterization time, postoperative hospitalization days, complications of two groups were compared. After the follow-up for 3 months, the changes of Qmax, PVR, OPSS, QOL were compared. Results The operating time, catheterization time, postoperative hospitalization days in PVP and TURP groups were (54.9±20.0) min and (45.1±18.0) min, (1.1±0.2) d and (3.4±0.3) d, (3.2± 1.1) d and (8.8±1.3) d, respectively. There were significant differences between two groups (P 〈 0. 05). After 3 months, 242 cases of PVP and 226 cases of TURP were followed up; Qmax, VR, IPSS and QOL improved significantly compared with preoperation, the differences were significant (P 〈 0.01). No significant difference was found compared between the two groups. Conclusion PVP is superior to TURP in safety and as efficacious as TURP.
