改良跟骨外侧L形切口手术治疗跟骨骨折 被引量:4

Improved calcaneal lateral L-shaped incision surgery for calcaneal fracture

摘要 目的探讨改良跟骨外侧L形切口的新型入路手术治疗跟骨关节内骨折方法及临床效果。方法2008年2月~2011年10月收治跟骨关节内骨折患者46例52侧,均采用改良跟骨外侧L形切口手术治疗,术中骨折复位后常规植骨,选择跟骨解剖型钛板内固定。结果随访时间6~18个月,伤口愈合良好,局部皮缘坏死2侧,其中发生感染1侧。骨折平均愈合时间8~16周,按Maryland足部评分系统进行功能评定,优良率为90.4%。结论改良跟骨外侧L形切口手术治疗跟骨关节内骨折能减少术后创缘皮肤坏死、不愈合,减少伤口不愈合引起的跟骨感染等并发症。 Objective To investigate the methods and clinical effects of improved calcaneal lateral L-shaped incision new approach for surgical in the treatment of calcaneal intra-articular fractures. Methods Forty six cases of patients (52 sides) with calcaneal intra-articular fractures admitted from February 2008 to October 2011, all received improved l^tteral cal- caneal L-shaped incision surgery, conventional bone graft after intraoperative fractures, then selected ealeaneal anatomy ti- tanium plate fixed. Results Follow-up for 6 to 18 months, the wound healed well, local skin flap necrosis occurred in two sides, in which one side of infection. The average healing time was 8 to 16 weeks, function was evaluated acco^ing to the Maryland Foot Score, the excellent rate was 90.4%. Conclusion Improved lateral calcaneal L-shaped incision surgery for callcaneal intra-articular fractures can reduce postoperative wound edge skin necrosis and nonunion, and reduce ealcaneal infection and other complications caused by wound nonunion.
