经鼻内镜行视神经管减压的手术配合体会

目的分析和总结经鼻内镜行视神经管减压手术配合特点,提高手术配合质量,减少手术并发症。方法回顾性总结33例（34眼）外伤性视神经病变患者采用鼻内镜下填塞肾水棉片,切除钩突,开放筛泡,清除全部前、后组筛窦气房,扩大蝶窦前壁,寻找视神经管,用钻磨削压迫视神经管的骨质,清除视神经管周围的碎骨片、填塞鼻腔等视神经管减压手术配合流程。结果 33例（34眼）视神经管减压手术顺利完成,效果满意,随访3~12个月,患者视力均有不同程度的提高,无一例发生严重的并发症。结论经鼻内镜行视神经管减压术创伤小、恢复快,是治疗视神经损伤较好的方法。充分的术前准备,正确的手术体位,高质量的手术配合是保证手术成功的关键。

Objective Analysis and summary the features of endoscopic optic nerve decompression surgery, improve the quality of surgery and reduce surgical complications. Methods Retrospectively reviewed 33 cases of taumatic optic neuropathy were treated with endoscopic packing of cotton piece of renal water, removal of uncinate, open bubble screen , clear all the before and after the ethmoid sinus air room, expand the sphenoid sinusanterior to find the optic canal, Drill grinding oppression of the optic canal bone, remove bone chips around the optic canal, nasal packing and other optic nerve decompression surgery with the flow. Results 33 patients unederwent the successful completion of the optic canal decompression surgery, Follow-up of 3-12 months, visual acuity improved in varying degrees, no serious complications occurred in.Conclusion Endoscopic optic nerve decompression line trauma, quick recovery, treatment of optic nerve damage is a better method. Adequate preoperative preparation, is to ensure high-quality operation with the key to successful operation.
