小剂量口服醋酸去氨加压素治疗老年男性夜尿

Low does oral Desmopressin in treatment of nocturia in elderly men

目的观察小剂量口服醋酸去氨加压素治疗老年男性夜尿的疗效和安全性。方法选择30例在2009年7月至2010年12月在北京同仁医院泌尿科门诊就诊的、以夜尿为主诉的老年男性。所有患者夜间排尿次数≥2次。入组患者每天睡前服用0．1mg醋酸去氨加压素片。疗程为8周。患者在治疗前和治疗4周、8周后均测量下列指标：夜尿治愈率，平均夜尿次数，平均第一睡眠周期时间及睡眠质量。结果治疗4周后，17例患者（56．7％）夜尿次数〈2次，13例患者（43．3％）仍然存在≥2次的夜尿。治疗8周后，22例患者（73．3％）夜尿次数〈2次，仅有8例患者（26．7％）仍然存在≥2次的夜尿（P〈0．01）。治疗8周后，平均夜尿次数由2．8次减少到1．5次（P〈0．001）。平均第一睡眠周期时间增加了60％（从2．5h增加到4h，P〈0．01）。治疗前仅20．0％的患者对睡眠质量满意，治疗后83．3％的患者对睡眠质量满意（P〈0．05）。结论小剂量口服醋酸去氨加压素治疗老年男性夜尿安全有效。

Objective To investigate the efficacy and safety of lowest dose of oral desmopressin in treatment of nocturia in elderly men. Methods Thirty old men who complained of about 2 voids or more per night were recurited in the study for treatment ofnocturia. The patients received 0.1 mg desmopressin at bed time for 8 weeks. Nocturia cure rate, mean number of nocturia, mean duration of the first period and sleep quality of patients were assessed after 4 and 8 weeks of treatment. Results After 4 weeks of treatment with desmopressin, 17 patients （56.7%） had less than 2 voids, 13 patients （43.3%） had more than 2 voids per night （P〈0.05）. After 8 weeks, 22 patients （73.3%） had less than 2 voids and only 8 patients （26.7%） had more than 2 voids per night （P〈0.01）. After 8 weeks, mean number of nocturia was significantly decreased （2.8 versus 1.5, P〈0.001）. The mean duration of the first sleep period increased by 60% （ 2.5h versus 4h） （P〈0.01）. Only 20.0% of patients were satisfied with their sleep quality before the treatment. After 8 weeks treatment, 83.3% of patients were satisfied with their sleep quality （P〈0.05）. Conclusion Low does oral administration of desmopressin is an effective and safe method for treatment of nocturia in elderly men.
