摘要
目的探讨男性不育症患者的精液参数和睾丸体积与中医证型的相关性。方法选择临床确诊的少弱精子症不育患者202例,分为肾阴虚组、肾阳虚组、气血两虚组、湿热下注组、肝郁气滞组和无明显症状组,进行精液常规分析,测定睾丸体积。结果肾阴虚组异常精子浓度和精子总数的比率,明显高于正常精子浓度和正常精子总数的比率(P〈0.01);肾阴虚组的精子浓度和精子总数显著低于其他证型组(P〈0.05)。肾阳虚组和气血两虚组正常精子总活动率的比率明显低于异常精子总活动率(P〈0.05);肾阳虚组和气血两虚组的前向运动精子率和总活动率显著低于其他证型组(P〈0.05)。睾丸体积异常者,均属于肾阴虚,肾阴虚患者的睾丸体积显著小于其他证型组(P〈0.01)。结论不育症患者的精液参数和睾丸体积与中医证型有一定的相关性,这为微观辨证和临床治疗男性不育症提供重要的参考价值和指导意义。
Objective To evaluate the correlation between TCM syndrome types and semen parameters and testicular volume in male infertility. Methods Two hundred and two cases of clinical diagnosed infertile patients were divided into 6 groups: kidney yin deficiency, kidney yang defficiency, qi and blood deficiency, down-pouring of dampness-heat, stagnation of liver qi, and absence of obvious symptoms. Routine semen analysis and testicular volume determination were done. Results In the kidney yin deficiency group, the abnormal ratio of sperm concentration and total sperm number was significantly higher than that in the normal group (P〈0.01). Moreover, the sperm concentration and total sperm number of the kidney yin deficiency group were significantly lower than that in other syndrome groups (P〈0.05). The normal ratio of total activity in the kidney yang deficiency and qi and blood deficiency groups was significant less than the abnormal ratio (P〈0.05). The progressive motility and total activity rate of the kidney yang deficiency and qi and blood deficiency groups was significantly less than that in other syndrome groups (P〈0.05). In comparison with the smaller testicular volume in the kidney yin deficiency group and the normal testicular volume in other groups, the former was much smaller than the latter (P〈0.01). Conclusion The semen parameters and testicular volume of male infertility are related to some extent to the TCM syndrome types. This study provides an important reference and guidance for microcosmic syndrome differentiation and treatment of male infertility.
