高脂血症性急性胰腺炎与胆源性胰腺炎临床对比研究 被引量:5

摘要 目的对比研究高脂血症性急性胰腺炎(hyperlipidemic acute pancreatitis,HLAP)与急性胆源性胰腺炎(acute biliary pancreatitis,ABP)的临床特征。方法回顾性分析2007年1月-2010年12月收治的HLAP 47例和ABP 55例患者的临床资料。结果 HLAP组体质量指数(body mass index,BMI),重症型、Ranson评分≥3分、CT分级D和E、APACHEⅡ评分≥8分比例均较ABP组高;与ABP组相比,HLAP组平均住院时间长,平均复发次数多,病死率高。HLAP组血清血三酰甘油、血糖、尿酸均高于ABP组,而血尿淀粉酶、血钙、丙氨酸氨基转移酶、γ-谷氨酰转肽酶及总胆红素、直接反应胆红素均低于ABP组(P<0.01,P<0.05)。结论与ABP组相比,HLAP组通常病情较重,常不伴有血淀粉酶的显著升高,住院时间长,易复发,积极降低血三酰甘油是治疗和预防复发的关键,重症者宜行血液净化滤过治疗。
