微创经皮肾镜取石术后泌尿道感染相关危险因素分析

Risk factors for urinary tract infections in patients after minimally invasive percutaneous nephrolithotomy

目的研究微创经皮肾镜取石术(MPCNL)后泌尿道感染的危险因素。方法对2010年MPCNL术后并泌尿道感染患者进行目标性监测,统计分析医院感染数据及危险因素。结果共调查478例MPCNL术后患者,发生泌尿道感染25例,感染率为5.23%,感染28例次,例次感染率为5.86%,病原体检出排前3位的依次为大肠埃希菌占25.37%、真菌12.5%、金黄色葡萄球菌3.68%;住院时间>7d、3个月内再次手术、留置导尿管、基础疾病、使用抗菌药物种类>3种且>3d是MPCNL术后泌尿道感染发生的危险因素。结论微创经皮肾镜取石术后需要针对性地采取多种措施,才能降低泌尿道感染的发病率。

OBJECTIVE To analyze the risk factors for urinary tract infections in patients after minimally invasive percutaneous nephrolithotomy（MPCNL）.METHODS By target monitoring of the inpatients who had received MPCNL in 2010,the risk factors were statistically analyzed.RESULTS Of 478 patients who underwent MPCNL,urinary tract infections occurred in 25 patients with the infection rate of 5.23%,28 case-times with the case-time infection rate of 5.86%;the most predominant pathogen was Escherichia coli（25.37%）,followed by fungi（12.5%） and Staphylococcus aureus（3.68%）;the hospital stay more than 7 days,operation again within three months,indwelling catheterization,underlying diseases,the use of more than 3 antibiotics for more than 3 days were the risk factors for urinary tract infections in these patients.CONCLUSION Multiple measures should be correspondingly taken to decrease the incidence of urinary tract infections in the patients after minimally invasive percutaneous nephrolithotomy.
