2型糖尿病中医辨证分型与血清视黄醇结合蛋白4、胰岛素抵抗指数的相关性研究 被引量:5

摘要 目的:初步探讨血清视黄醇结合蛋白4、胰岛素抵抗指数与中医辨证分型的关系。方法:将入组的300例糖尿病患者进行中医辨证分型,分为阴虚热盛型(58例)、气阴两虚型(177例)、血瘀脉络型(35例)和气虚痰浊型(30例)4组,并设健康对照组(30例)。全部病例监测空腹血糖(FPG)、空腹胰岛素(FINS)、计算胰岛素抵抗指数(IR),测定血清视黄醇结合蛋白4(Retinol binding protein,RBP4)。结果:①气虚痰浊型患者RBP4显著高于健康对照组、血瘀脉络型、阴虚热盛型与气阴两虚型;②RBP4与IR之间呈正相关(r=0.312;P<0.01)。结论:血清视黄醇结合蛋白4与气虚痰浊型糖尿病患者关系密切;血清视黄醇结合蛋白4水平与胰岛素抵抗呈正相关,降低血清视黄醇水平可能是改善胰岛素抵抗的一种新方法。 Objective:To explore the relationship between insulin resistance index,RBP4 and syndrome differentiation preliminarily.Methods:300 patients with diabetes mellitus were divided into four groups based on syndrome differentiation:the asthenia heat due to yin deficiency(58 cases),qi and yin deficiency(177 cases),blood stasis(35 cases),qi deficiency with phlegm(30 cases);normal controling group(30 cases).All cases were observed fasting plasma glucose(FPG),fasting plasma insulin(FINS),retinol binding protein(RBP4),calculatied insulin resistance index.Results:1.The group of qi deficiency with phlegm was apparently higher than the normal controling group,blood stasis group,asthenia heat due to deficiency yin group,the group of Qi and yin deficiency;2.RBP4 and IR were positively associated(r=0.312;P0.01).Conclution:There is close relationship between RBP4 and syndrome of qi deficiency with phlegm with diabetes mellitus and RBP4 positively associated with IR,so reducing RBP4 could be a new method of treating insulin resistance index.
