经颅多普勒检测颅脑损伤后早期脑血流动力学变化研究 被引量:5

摘要 目的研究经颅多普勒(TCD)检测轻、中型颅脑损伤患者早期脑血流动力学变化的作用及价值。方法选择40例急性轻、中型颅脑损伤患者作为观察组,40例健康者作为对照组。应用TCD分别检测2组的脑血流动力学指标,对检测结果进行统计学处理。结果观察组TCD检测脑血流异常35例(88%),其中28例(80%)表现为脑动脉血流速度增快,但与对照组相比无显著性差异(P>0.05);7例(20%)表现为血流速度减慢。14例(35%)出现血流搏动指数增高。结论 TCD可以作为轻、中型颅脑外伤后早期脑血流动力学变化的有效检测方法,能够为临床早期干预治疗提供直接可靠的依据。 Objective It is to study the function and value of transcranial Doppler(TCD) in the detection of cerebral blood flow in mild or moderate craniocerebral injury. Methods 40 cases of patients with mild or moderate craniocerebral injury were chosen as observation group, and 40 cases of healthy people were chosen as control group. The indexes of cerebrovascular he- modynamics were detected by TCD in both groups, and the detected results were analyzed with statistical methods. Results In the observation group, there were 35 cases (88%) whose cerebral flow were abnormal, in which 28 cases (80%) with rapid speed of cerebral blood flow, but compared with the control group, the difference was not significant (P 〉 0.05) ; there were 7 cases (20%) with slow cerebral blood flow and 14 cases (35%) with high pulsatility index. Conclusion TCD can be used as a effective method for detecting cerebrovascular hemodynamics after mild or moderate craniocerebral injury in early stage, and it can provides direct and reliable evidence for the early intervention and treatment for this injury in clinic.
