Chronic GVHD with nail involvement as predominant presentation

报告1例以甲损害为主要表现的慢性移植物抗宿主病。患者男,43岁。因指(趾)甲改变伴左足疼痛半年就诊。患者1年半前行姊供弟6/6相合异基因造血干细胞移植,血型A-A。半年前出现口腔白斑,视力下降及指(趾)甲改变。皮肤科检查:双侧颊黏膜可见环形白斑,口角溃疡。双手指甲可见纵向隆起及出血,双足趾甲变厚、表面粗糙、易断,部分远端缺失,左足大拇趾有触痛。

A case of chronic GVHD with nail involvement is reported. A 43-year-old male presented with nail changes of fingers and toes and pain of left foot for six months. He was a recipient of allo-HSCT （allogeneie hematopoietic stem cell transplantation with blood type A-A）, and the donor was his sister. One year after the transplantation oral lacy white plaques, diminution of vision and nail changes appeared. Dermatologie examination showed reticulate white lesions on the buccal mucosa, and ulcers of angles of mouth. The finger nails showed longitudinal ridging with subungual hemorrhage. The toe nails became thicknened, rough, and brittle and loss of distal part of some toe nails. The left foot was swollen, red and tender.
